MIRF REPORT ENTRIES

Pt attempted hand stand and collapsed to her head. No LOC no seizure activity. No signs of trauma.

Steam burn(first degree) received by North Coast food server approx 1500 hrs to L hand, approached medical due to persistent pain, given ice pack to alleviate pain and swelling, advised further actions ( otc pain relief spray, otc pain relief medicine as needed )

Saylor fell off of her chair (while seated). Medical was contacted to evaluate Saylors condition and recommend any further treatment. Upon arrival Saylor was alert and well oriented (age considered). Her pupils were equal and reactive to light indicating no serious head trauma. Upon palpating the head only a slight (if any) bump was felt on the left side. When Adam and Brooke arrived on scene they were told to monitor Saylor to see if she presented with “unusual” behavior as the evening progressed.

Child says she was outside on the preschool yard and was climbing near the wooden house, slipped and hit her head on the ground. Incident wasn’t witness but one of the volunteers found her. She was alert and oriented x 3 and crying. She states she is not in any pain. She has a 2×2 inch raised bump on the left side of her forehead. No bleeding. Ice applied x15 min. Mom called.

Child scratched accidentally in the face by another toddler. 1/8″ superficial cut on upper left cheek cleansed with sterile water and gauze. Amy Irving preschool staff to instruct parents to keep are clean and take to pediatrician if any redness or swelling or other signs of infection.

Ice applied by room Volunteers, assessment and comfort care provided.
