MIRF REPORT ENTRIES | North Coast Safety

Sat 2 Child was running through the hallway by the middle school/high school rooms when he tripped and fell onto his left knee. He was wearing croc shoes. Floor was not noted wet or damaged. Child was lying on the floor grasping his knee when I arrived. Shortly after patient was moved to a chair and examined. No significant trauma noted to his left knee. No deformity or bruising. He had full motion. Was able to ambulate normally after rest. Denied hitting his head & no LOC reported. No other trauma or pain. No back/neck pain

Sun 3 A 5 year old girl was getting herself some hot chocolate being served in a plastic coffee dispenser and spilled it on her right forearm and shirt. Her mom took her to the bathroom to put it under cold water from the sink. The house manager, Lindsey Marsolais, and a friend also helped. Lindsey called Safety over the radio and I met her by the bathroom. Safety got some washcloths to soak in cool water so the girl could keep them in her arm. When the mom brought the girl out, safety took a look. The girl was crying and her arm was a little pink but didn’t appear to be bad. Mom thought she was in a little pain and scared, but otherwise ok. The girl had also spilled the hot chocolate on her donut, which we replaced at her request. Mom took her home and was going to keep an eye on it. She said they would go to the doctor if it didnt get better today.

17 Medical was called as female adult was experiencing significant pain in her private area. She said it was probably a UTI as she experienced similar symptoms in the past. She wanted Tylenol. Found a pack of 500mg caps (2); she wanted only one but I let her keep the opened pack. She said she was visiting from Arizona and didn’t bring a pain med. She was very thankful for the assistance and support.

21 Patient tripped and fell at the North Entrance walking to the parking lot. Fell to left side landing on left hip and arm. Patient complained of left hip pain with a left hand deformity. Denies hitting head or having head or neck pain. Denies taking blood thinners. Patient does have a history of dementia and Parkinson’s but was at baseline orientation after fall.
