2	3	4	5	6	7	8 Felt dizzy & lightheaded, was seating in the outdoor venue Vitals: BP 110/80, HR 49, Oxygen sat 97%, Resp.rate 18. Bl sugar 113. He had cup of coffee prior to church. No prescription meds. Athletic, per him, runner. 5th grader in the 4th grade venue, bumped into another classmate face to face, playing game & hit nose causing bleeding.
9	10	11	12	13 Notified at 17:15 Patient reports pain to right side jaw due to patron named Trenton who bit her at approximately 18:15 – she verbalized she spoke with mother of patron and continued with event and now had pain. 19:20 While reviewing initial incident with patient , the patron walked over to patient and unable to be redirected by patient bit her on left shoulder. Security removed patron from area and was returned back to parent Update to 19:39 Director Marcia Boeche Per Juan: Mom reports Patient was walking and suddenly dropped and began seizing. (Patient has history of seizures, last seizure 13 yrs prior, recently changed anti seizure medications). Juan provided safety support by turning patient to side lying position. Writer observed: Bleeding to Left side of mouth due to left side lying position , noted cut to right side lateral tongue. Patient was able to call out for mother, and was smiling when he saw mother.	14	15 Fell backwards on fence, hit crossbar, no ground level fall. Fell into it onto upper T-spine, WNL, Moving extremities. No KO, ambulatory upon arrival. Light redness, administerd ice pack. States feeling better now that Dad here to pick up Patient was walking from the parking lot to the main campus with her husband, who has Parkinson’s disease. He lost his balance and fell on her. She reported pain in her right leg with an inability to bear weight. Medical was called to come assess and offer advice. India tripped on plastic chair and fell hitting the side of her face on the right side on the chair. Child was alert and oriented, child has cry a little. The area on the side of her face was pink in color, the size of her skin that was pink was less than a quarter. The skin was intact, no apparent swelling at this time.
