16 Fell off of a Barbie Scooter and bumped his head. Ice pack applied. Notified parents. (Mom) Fell off of a Barbie Scooter and bumped his head. Ice pack applied. Notified parents. (Mom) Chest pain and anxiety. Patient taken to Tracie‘s office in a wheelchair and monitored until 911 arrived Pt fell on to concrete surface. Neg loc, neg signs of trauma. Mother just wanted pt checked. Mom declined EMD services.

22 Shelf from cabinet in Café fell on right hand across proximal phalanges. Minimal edema and bruising noted. Good perfusion, range of motion limited due to pain. Ice pack and elevation. Instructed to seek medical treatment if any changes.

23 Pet patient she injured her eye with the hair brush this morning. R/0 corneal abrasion. Patched eye. It was red and tearing. Advised she take her own Motrin and recommend she go to urgent care so they can dye it and take a better look. Pt gave verbal understanding (she was a greeter this am) Right eye redness near medial pupil after getting hit I. Eye with a stuffed animal. Pt is complaining of pain. Unable to visualize any scratch however cannot rule out corneal scratch. Called for parent Jessica who I explained signs and symptoms of corneal issue and if so advised she see pediatrician. Per mother child has had bilateral eye surgery so “he is sensitive” Crawling and hit her head on the floor. Small red mark on forehead. Shannon Brandt RN, also conducted medical eval and advised mother.
