Incident Report / Actions Taken by Team Members

On 07/31/22, at 1100 hours, Charlene Macias was parked in her red Toyota Tacoma pick-up truck (Ca. 43068M1) in a handicap stall in lot C. She placed the vehicle in reverse and began backing. As she did so, she spilled coffee on herself. Macias braked, opened her driver’s door and began to exit the vehicle. However, the vehicle remained in reverse and knocked her to the ground. The driverless vehicle continued to back and the front tire rolled over Macias’ foot. The truck continued in reverse. The rear bumper struck the rear bumper/trunk of an unoccupied white Honda Accord (Ca. 6FLZ759) parked across the aisle.

Parking coordinators notified the safety team of the incident. Team Lead (Greg Flood) and Medical (Rene Schraeder) responded. Macias was ambulatory. She had an abrasion to her right arm and a complaint of pain to her right foot and groin. Nurse Schraeder bandaged Macias’ injured arm and conducted a physical examination. Macias declined to have paramedics respond. Nurse Schraeder recommended Macias seek follow up medical treatment.

There was minor damage to the trunk and rear bumper of the Honda Accord. Macias remained at the scene until the owner, Brooke Steires, returned. Steires works in the North Coast children’s ministry. The meeting was amicable and the two exchanged information. No further action from the safety team was required. STAR captured the incident on video surveillance.
