Describe Medical Event:

Patient was sitting in chair walker being pushed by patients son while heading into church service at rancho Bernardo campus on Patient fell onto concrete walkway face first from sitting position on wheelchair. Patient rolled over and remained in supine position with family until team arrived shortly after. Patient complained of bilateral knee pain, left hand pain, hip pain, and pain to face and mouth. No loss of consciousness. Per family, patient takes plavix for history of TIA. Left hand wound noted, with deep skin tear. Bleeding from lips and nose also noted. Skin tear noted on right knee.
