Describe Medical Event:
Zion was playing outside where there were a group of kids playing, volunteer found him crying sitting in the corner with blood on the lip. I evaluated Zion and bleeding had stopped but small red swelling noted on left lower lip. Ice pack applied. Scalp evaluated for swelling/lacs. Patient crying, dentention intact. Moving extremities equally and eyes tracking. Handed Zion off to parent Quentin who checked Zion out of classroom.
