Sun 1 Patient was hit in right eye with a soft, fake, snowball. Patient was hit in right eye with a soft, fake, snowball.

3 Neriah Easter passed out during worship in the Edge venue. Two other students/friends who knew Easter’s mother was on-site ran to get her. I was notified by Cassie Easter about the fall and entered the Edge with Cassie to find Neriah. A leader was attending to Neriah when we arrived. Neriah had already regained consciousness and was walked outside by her mother and the leader. While outside, Cassie checked her daughter for injuries. I observed her lower lip was swollen and had a deep laceration to the inside of it. She also had slight swelling above her left eye. Both pupils appeared normal. She did not complain of pain to her head when her mother felt it. Neriah did not remember what had happened. Friends provided some details to include Neriah stating her head was hurting and started slouching into one of the friends. Neriah said she wanted her mom. The friend started to walk Neriah away from the stage and towards the exit, when Neriah passed out. Several accounts stated she struck her head on the ground and was briefly unconscious.

8 Patient described experiencing a brief syncope episode while seated in a chair. Patient states that she has a history of previous syncope. Patient states that she has been under significant stress recently. The patient regained consciousness prior to medical team contact and complained of dizziness at time of assessment. BP 130/70 HR 70 RR 16 Patient noted that symptoms improved and opted to remain in Live venue for second service.

22 Patient complained of sudden onset of nausea, and dizziness. Patient stated that nausea/dizziness was unprovoked and started while walking onto campus. Patient has a history of being 32 weeks pregnant, and states that she has had episodes like this before. Child sitting on edge of carpet, leaned forward for toy, had witnessed fall from sitting position and right forehead hit floor. Volunteer worker Grace observed fall and big child up and immediately called me to evaluate her. Grace reports Child cried a bit for 30 to 60 seconds, then was readily consulted. NC teachers activated medical for Cyrus when he tripped and fell. The fall was not directly seen, but they heard him fall. They assumed that he hit his nose and face on the small table, or the table and the floor. He had no witnessed loss of consciousness, and was tracking and actively crying when medical arrived.

24 Madison felt faint. Has been on antibiotics x 5 days. She was slightly pale. Mucous membranes moist and pink. Radial pulse strong and regular.

29 Linnaea fell outside in the fenced in portion connected to the one year olds room. She fell backwards and hit her head. She had a small bump on the right side back of her head. She also had a minor cut to her lip from where a tooth must have cut into her lip.
