Thu 3 Baby tried to pull herself up using the corner of the wall in room 3 and slipped and bumped her forehead (left side). There was a large notable bump with bruising and a minor laceration. Ice applied immediately and mom was called. Mom will take child to the doctor if needed.

Sun 6 Jammed left fingers playing basketball Hit in the head with a hoola hoop. Given an ice pack Ice pack – referred to parents to seek medical fiolow-up

8 Child fell off of a skate board while skateboarding after Elevate Jr. High group and broke her arm. Dad was present when it occurred and immediately transported her to Palomar Hospital. Safety found out after the incident. Family are regular members and seemed ok with the incident other than being concerned for their daughter.
