Justin tripped and fell striking the left side of his forehead on cabinet corner.

Hadley fell and hit head on a rock outside. She sustained a small hematoma.

Theresa experienced a mechanical fall after tripping on a play center in the children’s ministry. Theresa fell to the ground and struck her left shoulder on the wall. Patient denied any loss of consciousness. Physical exam revealed tenderness to the left shoulder with no visible signs of trauma. Patient displayed regular use of the affected extremity with complete range of motion.

Called for nose bleed x 5 min.
Pt said he hit himself in the nose by mistake, and his nose started bleeding. He was not in pain and he was holding Kleenexes on his nose. He said he has had nosebleeds in the past, and they have stopped withholding tissue on the nose.
