Children’s worker Robyn Grinder witnessed the incident and confirmed it was an accident. Sun 6 Patient fell backwards off the top portion of the slide hitting the back of his head on the concrete. Witness fall by two staff members and activate medical. No loss of consciousness, medical encountered patient crying and acting appropriately. No signs of trauma on palpitation and visual. Has a history of vertigo. Patient fell from 1 step on in-room play structure. Nausea, dizziness. Mother reports patient started antibiotics day prior for skin infection to left arm, had taken this am with only banana. Mother provided plain bagel and soda/Sprite. Two sisters were in the sanctuary with Dad when they both turned towards each other suddenly by accident and Ellie’s head bumped Bridget’s lip. 21 22 23 24 25 26 Near syncopal episode while having a bowel movement. 27 Left hand with minor trauma to the middle finger and ring finger – result of having a door on a kids kitchen play area. She demonstrated good grip strength of the injured hand. GOOD PMS. Very small break of the skin on the two fingers. Minor bruising and swelling. 28 29 30 31
