14 While walking down the stairs Chrissy miss stepped on the stairs fell causing her glass drink water to shatter and cause a minor cut on her left hand. Minor cut is located between the index and middle finger.

19 Child C/O abdominal pain Child and sister state this happens frequently Client stated her lower front tooth is loose. She wanted me to pull it. But it wasn’t loose enough. So I told her that maybe it’ll come out tomorrow. Child said she hit her head in the bathroom Child C/O ‘tummyache’

27 Patient fell while walking into the plaza for service. She suffered a small skin tear/abrasion on her right elbow. She also stated that she hit her chin, but did not lose consciousness. Cut to right knee
