MIRF REPORT ENTRIES

Patient experiencing dizziness and collapsed from standing position. No loss of consciousness, no visible traumas. No allergies. Patient takes numerous medications but unable to name them. Patient took meds at 0800 with no food. History of triple bypass. Dizziness occurring without exertion. Dizziness slow on set and persistent. No chest pain. Pain scale 0 (1-10).
