MIRF REPORT ENTRIES

Fell off play structure ladder 1in bruise above right eye no open skin.

Near syncope while standing. That patient was assisted to a seat in the back of Live. The patient was found to be mildly hypotensive, all other vital signs stable. Patient described a history of orthostatic hypotension with multiple prior episodes. Patient was monitored for 10 minutes and given water with significant improvement in blood pressure and symptoms. Patient returned to her seat.

Called as student was experiencing pain @ back of neck, midline, no trauma, skin intact,; oriented, very articulate in her explanation, no strenuous activity or previous incidents for a long time, per student. States when she had it a long time ago, parents provided a heat pad. Checked for mobility of extremities & no concerns. Able to walk without difficulty.

Sawyer was struck by a fist to the left upper chest by a child who apparently has special needs. There was mild redness in the area that he was struck an ice pack given to Sawyer. Apparently, shortly after this incident Amelia was scratched by the same child and had a small abrasion to her right lower leg, and a Band-Aid was applied.

It was reported that sand was thrown in the childs eyes. She was able to self report. Child stated that her right eye is “bothering me a little bit.” The child was not crying.
