2 Child was being hugged by another child, when the other child let go, Adam fell back and hit his head on the floor causing a large bump to the back of his head. Evaluated, ice pack provided, and child’s father was advised to seek further medical attention if desired/needed.	3	4	5	6	7	8 Tripped bumped upper lip. Minor abrasion Pulled puzzles off a shelf and hit self in face. Unwitnessed. Checked dentition which was intact. Applied ice pack for 5-10 minutes Pulled puzzles off a shelf and hit self in face.
