MIRF REPORT ENTRIES

Left 3rd 4th and 5th finger slammed in door. Full range of motion, normal cap refill. Small amt blood under L 4th fingernail. All 3 digits painful, slight edema.

pt reports she “did the splits after slipping on water” which she later believed was a grape on the floor of the kitchen. as a result she “twisted” her left ankle and fell primarily on her Right hip/thigh area, and broke her fall with her right hand. pt reported pain in left ankle, right wrist and right hip/thigh area. when I arrived on scene she was sitting on the floor and had her left boot off. She was being helped by Lowell who had called for assistance.
