Playing basketball, had ball and fell while running away from other child. Fell & twisted left knee inward. Escorted outdoors by boys Director of tower 6. While in tower 6, felt dizzy, and couldn’t see, started emesis outdoors just prior to bathroom. Vomited large amount of food content, & was witnessed by Gary (lead). She then felt better. 3 4 5 6 7 8 9 Patient states hit the back of his head under the fousball table when he stood up to soon. 15 y/o female patient was with mom in the main sanctuary when she became weak and lethargic. Mom walked her out to lobby. 10 11 12 13 14 15 16 17 18 19 20 21 22 Playing volleyball in classroom when Lance hit the ball with his palm his index & middle finger were bent and hit Wyatt above the eye but the ball hit Wyatt directly in his eye. Wyatt complains of pain in his right eye and that it is blurry when he looks far away. He was able to read close up. No swelling present but there is mild redness around the entire eye. Able to correctly ID # of fingers held up initially and again 10 min after treatment. Describes some pain behind eye. Lance c/o mild pain in his fingers. Able to easily flex and extend them without increased pain. Able to make a tight fist. 23 While going down the steps, getting to Live, she said: “my legs gave up”. She was wearing high platform shoes. Skinned both her mid-shins & hurt Rt. Elbow. No oozing, mild bruises in her shins. No markings on Rt.elbow.Has clean health history and takes no medications. Walked out of warehouse as was feeling nauseated and dizzy. Upon my arrival she was seated at coffee house entrance. Stated had a small piece of bread at home and was drinking water. During my assessment she threw up clear content & mucous. 24 25 During the 8th grade mid-group game, Ethan fell and another person fell on top of his arm, near the elbow. Ethan had slight pain just below the elbow and had difficulty straightening his arm. Ethan informed us that both of his arms were broken in Sept 2024 and he had surgery to correct them. 26 27 28
