MIRF REPORT ENTRIES

I was asked by Tytus’ father Caleb (NC Facilities) to check son about 20 mins after he fell off high wooden chair in Study Hall and struck posterior scalp on cement floor. I evaluated Tytus with dad Caleb present in Tytus’ class. Exam normal other than minimal soreness and very small scalp hematoma posterior occipital scalp. Neck nontender with full range of motion, normal eye exam and observed neuro function. Child smiling, communicating normally, in no apparent pain.

Toddler fell off play structure/ toy, right unto floor and hit forehead right area. Small goosebump, skin intact, held by mom.

Fall from bike and striking the back of his head on the concrete.

Patient tripped on a towel and slipped on the wet floor near the exterior door to the playground. Fell x 2.
