Sat up to run and tripped on carpet.
Slight nose bleed.
Repeat epistaxis, minor abrasion on top of nose, no active bleeding

Pt appeared weak, sweaty, dizzy, non responsive per witness. Upon assessment pt was diaphoretic, pale, alert/oriented to self, time, place, event. No slurred speech, no blurry/double vision, able to read full paragraph (no dysphasia) PERRLA, symmetric smile, raised brow, equal strength in arms held out for 10 seconds against gravity.
1032 reassessment- color returned, lightheaded/dizziness subsided, diaphoresis subsided,
Pt received hydration, family notified, Wheelchair escort to car offered.
