On Thursday, January 28th, 2021, I served as the Team Lead for the Jordan service in the plaza of the Vista campus. Alan Polfer was Security and Tom Bentley served in STAR. At roughly 2105 hours, we went off duty with no noteworthy incidents. At 2150 hours I received a call from Guardwest Security Officer Chris Craig reporting that a woman had slipped and fallen in the women’s restroom next to the Family Room of Building 5.

I responded to the Vista Campus and contacted Megan Cunningham. Megan was seated in the backseat of a vehicle parked next to the west pedestrian entrance of building 5. Megan was applying ice to her right knee. Megan told me she attended the Jordan service earlier in the evening. At around 9:40 pm, Megan and Emma Denton walked into the women’s bathroom. Megan entered a stall and slipped on the wet surface causing her to twist her knee and fall to the ground. She said she heard her knee pop and it began to swell up. Paramedics responded, assessed her injury and helped her out of the bathroom. She decided not to seek further medical attention that evening but would follow-up the next day. Megan was able to walk under her own power to the vehicle she was in. At the time of her fall, Megan was wearing Ugg boots.

I spoke to Emma Denton who told me she walked into the bathroom with Megan. Emma said as Megan entered a bathroom stall, she caught a glimpse of Megan slipping and falling to the ground. Megan told Emma she had injured her right knee from the fall and confirmed there was water on the floor in the area of the floor drain. I asked Emma to show me the stall where Megan had fallen. We walked into the women’s bathroom and Emma directed me to the third stall from the door. There is a drain just inside the stall on the right side. The flooring is a non-slip surface consisting of small tile squares with grout between each tile. I did not notice any water near the drain or anywhere else in the stall.

On Friday, January 29th, I reviewed video footage of the walkway leading to the restrooms of building 5. At 2135 hours, the video footage shows what appears to be Megan and Emma walking from the plaza area to the bathroom. At 2138 hours, Emma is seen running out of the restroom toward the area of the plaza. At 2147 hours, paramedics arrive and enter the restroom. At 2150 hours, paramedics assist Megan out of the restroom. Megan then walks with a noticeable limp to the west entrance and out of view.
